Programs for Minors Application Checklist

This application checklist is intended for any program, event or camp sponsored and operated by NCCU with the same group of minors without an overnight stay or that involves an overnight stay

• All forms listed below are required for a program to be in compliance with the NCCU Programs for Minors Policy (REG 80.07.1) and the University of North Carolina System Policy (1300.10). Remember that all forms require original signatures.
• Programs and Camps for Minors applications and required forms should be sent to [email protected].
• The Programs for Minors Application and Forms are located on the Youth Programs and Camps page.

Please note: The Programs and Camps for Minors Application and submission of required documents should be initiated 60 calendar days prior to your program or camp start date.

Submit the Programs for Minors application and the required forms/materials listed below for your program/event to [email protected]. Include appropriate signatures to get final approval for your event. 

• Programs and Camps for Minors application
• Program itinerary/agenda showing beginning and ending times of each activity
• A copy of the program flyer, brochure or website print-out
• Camp insurance application for your program
• A list indicating the NCCU staff, volunteers, and NCCU student volunteers who will be working for the program or camp
• Submit volunteer information via the Volunteer Waiver Form (required for all NCCU, volunteers and NCCU students)
• Ensure you have submitted the name, email address and phone number of all program, event or camp staff and volunteers to [email protected] to initiate the background check process with Human Resources.

After the above documents have been submitted and processed, please submit the following materials:

• Participant roster
• Waiver, Indemnification and Medical Treatment Authorization form for each camp participant
• Health Information, Photo Release, and Pick-up Authorization Form for each camp participant
• Medication Disbursement Authorization Form for each camp participant, as needed
