North Carolina Central University cares a great deal about the experiences of students, employees and members of the public who engage with our community. The safety of our campus and the individuals who work and learn within the campus environment are paramount. All members of the community are encouraged to report accidents, injuries or other incidents that may occur to University officials as soon as possible for investigation and resolution. Electronic reporting forms will be routed to the appropriate University department.
Title IX Reporting Form: Please use this form for reports of sexual misconduct, sexual or gender-based harassment, sexual assault, domestic violence, dating violence, sexual exploitation, stalking, and related forms of misconduct).
Campus Security Authority Crime Report Form: This form should be used by campus security authorities (CSAs) to report crimes under the Clery Act.
Incident Reporting Form: Please use this form for reporting student behaviors violating the Student Code of Conduct, whether these behaviors occur inside or outside the classroom setting.
Behavioral Intervention Team (BIT) Reporting Form: Please use this form to connect students who may be exhibiting concerning behavior with a continuum of care. Examples of concerning behaviors include self-injurious behavior, suicidal ideation or attempt, distressing, and disruptive and/or disturbing behavior, inside or outside of the classroom setting.
Workers’ Compensation Employee Statement Form: Supervisors should provide all injured employees with these forms to complete the information concerning the accident/incident and use of leave options for any time lost from work that may result from injury.
Grievance Packet for SHRA Employees: These forms are designed for grievances filed by SHRA employees, which may include allegations of harassment, discrimination and retaliation.
