RCMI Forms
Community Engagement Core Service Form
Who Should Submit: Submissions are welcomed from community-based organizations, researchers, or research teams dedicated to addressing community needs and advancing health equity.
Purpose: To outline the process for requesting services and collaborating with the CEC team. This ensures we have the necessary information to develop effective strategies for reaching and engaging communities, ultimately advancing health equity.
Consultation & Support: Upon submission of your form, you will be scheduled for a 45-minute consultation to review and clarify the scope, logistics, and any specialized support required (e.g., mental health, nutrition). The CEC Team will evaluate the feasibility of providing such support and will connect you with the appropriate experts.
Completion Review: Following the consultation, you will receive a summary, along with any additional forms necessary to finalize your planning. This ensures that the CEC Team’s support aligns with the event’s needs and scope.
Post-Event Form
Who should submit: The event coordinator or a designated person.
Purpose: To gather feedback and assess the event’s overall success, capturing insights on what worked well and identifying areas for improvement, as well as keeping track of our work for use in the newsletter and reports.
- Event Details: Include essential information such as the event date, location, and NCCU event coordinator.
- Engagement, Services and Photos: Estimate the number of attendees engaged, describe services provided, and upload any event photos to visually document the event.
- Feedback and Improvements: Summarize accomplishments and challenges and provide suggestions for future improvements, including feedback on event logistics and forms.
Van Request Protocol
Who should submit: Any NCCU staff or faculty member involved in Julius L. Chambers Biomedical/Biotechnology Research Institute (BBRI) projects who needs transportation for an event or project.
Purpose: To outline the process for requesting the use of university vans, ensuring proper scheduling, availability, and accountability while keeping a record of transportation needs for documentation and reporting.
- Complete the required driver training course (NCCU Driver Safety Program).
- Submit the Driver Authorization form.
- Read the new protocol on the use of university vehicles (BBRI).
- Provide a copy of your driver’s license.
- Complete the HR Background Check Authorization form. All of these steps must be completed before your van request can be secured.
Mobile Health Unit Request
Who should submit: Any NCCU staff or faculty member involved in health-related initiatives or community outreach programs.
Purpose: To facilitate the scheduling and deployment of the mobile health unit for health screenings and educational events, ensuring that all necessary resources are organized and available for effective service delivery.
- Only university-appointed drivers, not faculty or staff, will operate the mobile health units.
- The department reserving the vehicle is responsible for ensuring that eligible drivers are identified to operate the vans and mobile health units. Please schedule this in advance, with a minimum notice of 10 days.

Who should submit: Any NCCU staff or faculty member involved in RCMI projects or partnerships who has updates, achievements, or events to share.
Purpose: To communicate important information, highlight accomplishments, and share upcoming events within the community, helping to keep everyone informed and engaged while documenting our initiatives.
