Please only sign up if you are available to be vaccinated on March 31 and April 1 during the times noted.

NCCU students in Group 4 now qualify to be vaccinated and were provided with a separate sign up link.

PLEASE NOTE: By completing the form, you agree to have your name and email submitted to the COVID Vaccine Management System (CVMS) vaccine management portal and to be contacted to schedule an appointment time.

Late Arrival Procedures:

Patients are asked to arrive to their appointments 5-10 minutes before their scheduled appointment time. This allows enough time for the registration process to be completed before the actual appointment time. If a patient arrives more than 15 minutes late for their appointment, the patient will be given the option of either being seen that day as a walk-in, if the schedule permits, or rescheduled for a later date. This process will ensure patients that do arrive on time are seen in a timely manner.
