NCCU Business Travel and Study Abroad Application and Enrollment Form
This Enrollment Form is for international travelers to enroll in GEOBlue travel insurance when traveling on NCCU business.

NCCU Auto Accident Reporting Form
Use the NCCU Auto Accident Reporting Form to report all accidents involving state-owned vehicles.
NCCU Drone Insurance Request
Departments and units on the NCCU campus must obtain unmanned aerial system (UAS) insurance to cover their proposed UAS or drone activities. For more information and to access the UAS Pilot Registration form, please refer to the UAS information.
NCCU New Auto Insurance Report
Use this form to request insurance coverage for any state-owned self-propelled vehicles (riding mowers, golf carts, utility vehicles and automobiles). All fields are required for submittal. Do not operate the vehicle until you have confirmed insurance coverage.
NCCU Property Loss Reporting Form
Use this form to report property loss (building, contents and equipment). Provide as much information about the event as possible, as this information will be used to generate a property claim with the Department of Insurance. Please note that the deductible for all property claims will be $5,000 per occurrence.
NCCU Rental Vehicle Certificate Insurance Request
Use this form to request insurance coverage required to reserve a rental vehicle for state travel. Remember to use the state-contracted companies Hertz or Enterprise when making reservations.
NCCU Scheduled All-Risk Insurance Equipment Listing
Use this form to request all-risk coverage. All-risk coverage is available for high-value computers and miscellaneous equipment (i.e., video and sound equipment). All-risk coverage is primarily used for items that are mobile and cannot be included in standard property content coverage. The deductible for this coverage is $5,000 per occurrence.
Liability Waiver, Assumption of Risk and Indemnification Agreement
This liability waiver form requires members of the public to sign to minimize exposure to NCCU in the event of injury or property damage. This form should be used in connection with a University program or event.
